Indications

epilepsy:

• complex or simple partial seizures (with or without loss of consciousness) with or without secondary generalization;
• generalized tonic-clonic seizures;
• mixed forms of convulsive seizures.

Finlepsin® 400 retard can be used as monotherapy or as part of combination therapy.

Acute manic states; maintenance therapy for bipolar affective disorders to prevent exacerbations or to reduce the clinical manifestations of an exacerbation.

Alcohol withdrawal syndrome.

Idiopathic trigeminal neuralgia and trigeminal neuralgia in multiple sclerosis (typical and atypical).

Idiopathic neuralgia of the glossopharyngeal nerve.

Compound

active ingredient: carbamazepine; 1 tablet contains carbamazepine 400 mg Excipients: ammonium methacrylate copolymer (type B) dispersion, triacetin, talc, methacrylate copolymer dispersion, crospovidone, colloidal silicon dioxide, magnesium stearate, microcrystalline cellulose.

Contraindications

Finlepsin® 400 retard should not be administered:

• with established hypersensitivity to carbamazepine or chemically similar preparations (tricyclic antidepressants), or to other components of the preparation;
• with atrioventricular blockade;
• patients with a history of bone marrow suppression
• patients with a history of hepatic porphyria (eg, acute intermittent porphyria, mixed porphyria, tardive porphyria of the skin)
• in combination with MAO inhibitors (MAO) and within 14 days after stopping their intake;
• concomitantly with voriconazole, since treatment may be ineffective.

Dosage and administration

Treatment with carbamazepine should be started at low doses, which should be adjusted individually depending on the clinical condition of the patient. Later, gradually increase the dose until the optimal maintenance dose is reached.

In individual cases, the required dose may differ significantly from those doses that are used at the beginning of treatment, and maintenance doses (for example, due to increased or decreased disintegration of the preparation under the influence of inducing enzymes or substances that exhibit an inhibitory effect during combination therapy).

Finlepsin® 400 retard is prescribed orally, usually the daily dose of the preparation should be divided into 2 doses. You can take the preparation during meals, after meals or in between meals with a small amount of liquid (for example, a glass of water).

Finlepsin® 400 retard tablets can be divided in half. Tablets can be dissolved in water, since after dissolving in water of tablets of prolonged action, the properties of prolonged action are retained in suspension.

The maximum daily dose (in exceptional cases) should not exceed 1600 mg due to the more intense occurrence of adverse reactions after taking the preparation in high doses.

If the immediate release dosage form is changed to Finlepsin® 400 retard extended-release tablets, it should be ensured that the appropriate plasma concentration of carbamazepine is achieved.

Under no circumstances can the patient arbitrarily stop treatment.

Patients of Han Chinese ethnicity or those of Thai origin should, if possible, be screened for HLA-B*1502 before starting treatment, as this allele may precipitate severe carbamazepine-associated Stevens-Johnson syndrome.

Treatment begins with a low daily dose, which is then slowly increased (adjusted according to the needs of each individual patient) until the optimal effect is achieved.

In cases where possible, Finlepsin® 400 retard should be administered as monotherapy, but in the case of use with other preparations, a regimen of the same gradual increase in the dose of the preparation is recommended. If Finlepsin® 400 retard is added to existing antiepileptic therapy, the dose should be increased gradually, while the doses of the preparations are used, not changed or, if necessary, adjusted.

Treatment should be under the supervision of a specialist.

To select the optimal dose of the preparation, it may be useful to determine the level of the active substance in the blood plasma. Therapeutic plasma concentration should be 4-12 µg/ml.

In some patients, when using retard tablets, it may be necessary to increase the dose of the preparation.

adults

The recommended initial dose is 100-200 mg 1-2 times a day, then gradually increase the dose until the optimal effect is achieved; usually the daily dose is 800-1200 mg, should be divided into 2 doses. Some patients may require a dose of Finlepsin® 400 retard, up to 1600 mg or even 2000 mg/day.

Elderly patients

Taking into account preparation interactions and different pharmacokinetics of antiepileptic preparations, doses of Finlepsin® 400 retard should be selected with caution in elderly patients.
